What is ACCTiVATE? 

ACCTiVATE stands for Achieving Chronic Care Equity by Leveraging the Telehealth Ecosystem and is funded by the National Institutes of Health (NIH) / National Institute on Minority Health and Health Disparities (NIMHD). The study aims to support clinics to bridge telehealth digital divide by providing direct patient support and practice facilitation, in order to improve telehealth access and chronic disease management for patients with diabetes.
