Dr. Sharma is an Assistant Professor of Family and Community Medicine at UCSF, faculty at the Center for Excellence in Primary Care (CEPC), and mentored faculty at the UCSF Center for Vulnerable Populations. Dr. Sharma is an internationally recognized expert in patient engagement strategies for outpatient quality and safety and patient-engaged research. Dr. Sharma’s direct work with patient advisory councils generated the idea for this proposal, by gathering patient and stakeholder insights about how direct patient support and training was greatly needed across the San Francisco Health Network. Dr. Sharma has a highly successful track record of collaborating with the investigative team, including two manuscripts with Dr. Tuot and fourteen manuscripts published with Dr. Sarkar and/or Dr. Lyles. Of relevance to this proposal, Dr. Sharma has identified best practices and the demonstrated impact of advisory councils, specifically with how patient advisors can inform clinic-level practice transformation (PMID: 28076261, 28034916, 29058625). Dr. Sharma has collaborated with Ms. Willard-Grace in the above research, as well as focusing on health coaching for chronic disease management including hypertension and diabetes outcomes at the CEPC (PMID: 27184989). Dr. Sharma has expertise in observational studies, mixed methods, and qualitative analysis. Additionally, Dr. Sharma is Program Champion of telemedicine at her clinical practice site, with extensive experience implementing, adapting, and evaluating telemedicine and practice improvement for diverse populations. Dr. Sharma is a fluent non-native Spanish speaker, one of the target languages in this study, and we anticipate Dr. Sharma to be in close communication with the bilingual Spanish speaking health coach//navigator and members of the Stakeholder Advisory Council.
